About The Position

Our Brisbane North Community Aged & Disability team based in Stafford, is key in providing care to a range of clients, including older people and those with disabilities. Our teams provide support and services that enable our clients to maintain their independence - living happily and healthily within their own homes. We recognise that one size does not fit all, and working together is the key to great outcomes, so our non-clinical lifestyle programs are designed to support individual needs, incorporating the principles of social inclusion, choice and control, empowerment and independence; and our clinical supports include nursing and allied health care at home, along with domestic and personal assistance.

We are currently looking for a confident, collaborative and caring professional who will be responsible for engaging in client relationship management and packaged care case management with clients of consumer directed care service models. The Client Liaison role undertakes assessment, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client to achieve their identified goals. In addition, this role supports the client to manage and monitor their budget to ensure care packages are financially sustainable, and monitors the services provided to ensure these are in accordance with relevant service models and guidelines.

 

About You

With a background within Community Services, Aged Care or Disabilities sectors, you will offer the following experience:

  • Proven understanding of assessments, packaged care and case management
  • Ability to engage clients and work effectively with them to achieve identified goals
  • Support and guidance to clients with budget management and financial sustainability
  • Experience in monitoring and assessing services provided to ensure they are in accordance with service models and guidelines
  • Exceptional communication skills – both written and verbal – and the ability to relate to a wide range of people
  • Ability to advocate strongly for your clients and the services they require
  • A sound understanding of the community care system and the people who utilise this service
  • Diplomacy and negotiation skills; people management capabilities

Along with a Certificate IV in Health, Community Services, Aged Care or Disability Support, you will be required to hold current Blue Card (Working With Children) with Yellow Card exemption, a current Queensland Driver Licence and a National Police Certificate. Qualifications relating to Case Management would also be an advantage.
